Abstract

Endodontic therapy has become increasingly predictable due to significant scientific and

technological progress. Yet, the complexity and variability of root canal anatomy continue to

present daily challenges for clinicians. A thorough understanding of this anatomy is essential for

achieving success in both primary treatment and retreatment. In primary cases, outcomes largely

depend on the clinician’s ability to negotiate intricate canal systems, a task made more

reproducible with the advent of flexible, heat-treated NiTi files. Retreatment cases, however, often

prove even more demanding, requiring the management of iatrogenic complications such as

ledges, missed canals, fractured instruments, or perforations. This presentation will highlight the

realities clinicians encounter in managing endodontic cases, from routine to highly challenging

situations. Emphasis will be placed on strategies, modern tools, and evidence-based protocols that

allow practitioners to overcome obstacles and enhance the predictability of treatment. By

deepening the understanding of canal complexity and providing practical solutions, the ultimate

goal is to empower clinicians to deliver more consistent and successful endodontic outcomes.
